Fatal tuberculous meningitis in an infant presenting with seizures in the UK

A 5-month-old male child of European background presented with sudden onset prolonged afebrile seizures. He was intubated and transferred to the paediatric intensive care unit where he displayed abnormal neurology remained ventilated. Brain MRI showed basal leptomeningeal enhancement suggesting malignancy or infection. Subsequent cerebrospinal fluid results lymphocytic pleocytosis raised protein were suggestive tuberculous (TB) meningitis anti-TB treatment commenced empirically. Positive TB microbiology eventually confirmed on respiratory secretions. The infant continued show neurologic findings repeated neuroimaging a new extensive cerebral infarct. compassionately extubated passed away. father later found have pulmonary TB. This case is an important reminder for countries uncommon. importance considering in any taking prompt family histories identify children at risk highlighted.

Six cases of tuberculous meningitis, all occurring in patients over the age of 65, are described. All patients presented with an acute illness, primarily with a confusional state.
